Job Purpose To work autonomously within a multidisciplinary team to holistically provide and evaluate the nursing and rehabilitation needs of the patient following a stroke. To participate in the treatment and assessment of patients of patients within the Stroke specialty who have complex and/or chronic health needs. Working as an autonomous practitioner, you will identify the need for, and the implementation of, nursing assessment and intervention. To plan, deliver and evaluate the specialist nursing and rehabilitation programme in partnership with multi-disciplinary team (MDT) colleagues, including liaising, supporting and referring on to health and social care services, voluntary and public sector agencies. Dimensions To work in a variety of community settings which may include the patients own home/care/nursing home, setting up assessment and rehabilitation plans for patients, carers and families. Key Responsibilities Be responsible for the assessment of the health and wellbeing needs of patients and their carers within the Community Stroke Service, to identify nursing needs. Contribute to the public health agenda by raising awareness of Stroke and Stroke prevention, and promoting health and wellbeing to individuals. Plan, deliver and evaluate programmes of nursing intervention and rehabilitation to address patients health and wellbeing needs. Have a working knowledge of related medication and liaise with medical teams if needed. Implement the provision of evidence-based /best practice. Actively encourage patient and carer feedback about the Community Stroke Service Maintain a high standard of care to patients and carers Maintain a professional approach as outlined in the NMC code of conduct Contribute to the continued development of the service Contribute to the clinical governance agenda and apply risk management systems Participate actively in the services training strategy including in service training and multi-Agency training and supporting students by providing specialist stroke education. Contribute to the clinical audit process or service evaluation and research within the practice setting. Regularly take responsibility for the setting , reviewing and evaluating of appropriate nursing goals within the team Occasionally, in the absence of the Band 7s, take responsibility for the appropriate delegation and supervision of work according to the skills and abilities of the team members. Take responsibility for participating in the role of Duty clinician Promote and maintain good practice in record keeping by self and team to ensure professional and local standards are met. Support effective team working Be responsible for the development of owns knowledge and clinical practice and recognise ones own limitations making senior staff aware Act as a mentor to a variety of nursing students and clinical support staff Participate in the development of other team members Demonstrate continued professional development and support the registration renewal process. Contribute to the implementation of service developments Utilise enhanced knowledge and skills to promote continuous service improvements Promote and maintain best practice that ensures the health and safety and security of self, patients, and colleagues according with the policies, procedures and guidelines of the organisation. Follow IG guidelines and ensure confidentiality at all times. Practice in accordance with procedures and policies for the protection of vulnerable adults Undertake any additional duties as appropriate and delegated by the Team Manager, Senior Clinicians
