Crynodeb

Arrange and support completion of the electronic Holistic Needs Assessments (eHNA) during the cancer patient pathway. This will include the navigator assisting the patient with completing their eHNA where appropriate. This will ensure that patients are signposted in a timely manner to appropriate services, be responsible for keeping all HNA assessments up to date electronically and maintain databases. Provide support to the Programme Delivery Manager, in responding to the non-complex patient needs, and where appropriate make necessary referrals to other health care professionals. Assist in the development of end of treatment summaries alongside the programme manager to ensure the timely communication of patient information to the primary care setting. Assist with the planning and delivery of health and well-being events. Organise and prioritise own workload according to identified needs. Communicate effectively (daily and directly, face-to-face and telephone) with a range of individuals on various levels, e.g.: - patients, relatives, carers, other health / social care professionals and community workers. Work with the Programme Delivery Manager in promoting equity for patients across Coventry and Warwickshire. Assist in capturing cancer data on all patients on the eHNA system and assist in audit of this data. Assist in maintaining and developing databases and monitor the processing of this data. Assist in the development of care pathways and implement improvements to the administrative element of the pathway. Ensure that administrative and clerical functions are carried out to the highest quality standards by monitoring and improving services. To participate and display a willingness to learn develop any relevant technical competency/area of interest and keep up to date with current local and national issues and developments. To be responsible for own learning in knowledge base relevant to own clinical area including mandatory training. To participate in regular formal supervision and develop a personal development plan and departmental competency frameworks to promote learning and enhance skills. To be responsible for managing own daily timetable of delegated tasks including time for supervision and training To undertake any such duties as may be required from time to time, that are consistent with the responsibilities of the grade. To comply with the Trusts and departments policies, guidelines, and procedures. To attend all mandatory training as required in line with the Trust employment and professional need. To participate in team meetings. To be responsible for informing the line manager of any issues relating to the ability to safely carry out duties. To be aware of departmental duties and initiatives to ensure a safe environment, recognising patients requirements for privacy and dignity. To recognise and comply with the Trust and departmental policies and procedures for incident reporting. To always ensure confidentiality in accordance with Trust policy and procedure.
