Summary
Patient Flow Team Lead
Band 7 - £43,742 - £50,056 per annum
37.5 hours per week
Base - Negotiable
The post holder will be responsible for leading safe, resilient, and consistent team management and facilitating safe and effective discharge in line with the Trust policies and procedures, supporting the planning and co-ordination of patient flow across the care pathways. This will involve working in collaboration with the ward teams, key partner organisations, service users, families, carers, and other key agencies to ensure meaningful admission and safe, appropriate discharge. This is a senior leadership role and is focused on the quality of patient experience through delivery of high quality, safe and effective care and the post holder will demonstrate the Trust values and behaviours.
The post holder will be responsible for providing specialist advice and support to the wards and their multi-professional teams and community services in managing effective patient discharge as an integral part of care management across the Trust.
Responsible for maintaining a collaborative working interface with Community and Home Treatment Teams in order to identify patients suitable for discharge, and to work together to promote a smooth discharge pathway from inpatient services.
To work with the integrated flow team and other services to identify practical help that can be provided to clients in achieving a timely discharge.
To work as part an effective member of the multi-disciplinary flow team, leading and actively role modelling high standards of care.

1. Work in partnership with the ward teams, external services (e.g., social services, community, and voluntary services), and commissioners to support patient flow from admission to discharge, coordinating seamless and timely packages of care, geared to the needs of the service user.
2. Support the wider inpatient and community MDT to identify potential barriers to admission/discharge and how these can be resolved at an early stage to prevent unnecessary delays.
3. Lead the patient flow team, being responsible for identifying potential complex discharges on admission, and in collaboration with the multidisciplinary team set and monitor discharge dates to assist the Trust in meeting its NHS targets.
4. Provide direction, guidance and advice to ward based discharge co-ordination team, maintain oversight of progression.
5. Hold line management responsibility.
6. Have a working knowledge of the various funding streams and options to support effective discharge and the assessments and processes required to access them.
7. Negotiate funding with Clinical Commissioning Groups for home care packages and placements for patients meeting the criteria for fully funded NHS continuing health care.
