An exciting opportunity has arisen to join Lancashire and South Cumbria Foundation Trust as a Band 6 Physiotherapist within the Community Rehabilitation Team.



We are a team of Physiotherapists and Occupational Therapists working closely with the multidisciplinary teamto prevent further deterioration in patients’ physical functioning and wellbeing. We cover the areas of Chorley, Preston and South Ribble.



The post offers excellent personal and professional development within a well-established and supportive environment. The service works closely with Lancashire Teaching Hospitals and may include a potential rotation into the acute, which may be set up in the future.

There may also be the opportunity to work in different areas of the Community Rehabilitation Team dependant on service priorities.









Provide a high standard of specialist physiotherapy input to the designated therapy team.

To assess and treat patients within a ward or community setting as an autonomous practitioner

Physiotherapeutic assessment of patients with diverse presentations and complex physical and psychological conditions, to provide a diagnosis and develop and deliver an individualised treatment programme.

Hold responsibility for own complex caseload and be responsible for a defined area of the service or a particular patient type, working without direct supervision.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
