An exciting opportunity has arisen to work within our Community Therapy Team at Wyre Enhanced Primary Care.

Wyre Enhanced Primary Care consists of Physiotherapists, Occupational Therapists, Technical Instructors, Community Matrons, Care Home Team, Integrated Care Co-ordinators, Health and Wellbeing Support workers and Neighbourhood Assistants, all working together to meet the needs of our patients.

As the Band 7 Clinical Specialist Physiotherapist you will be required to plan, coordinating , deliver and evaluate the physiotherapy service to the patients within Wyre.

You will also work with other members of the multi-disciplinary team in order to provide a patient centred service. As a team we also work alongside our primary care colleagues, as we develop our integrated neighbourhood teams.

The post holder should have significant community and NHS experience. We are committed to education, continued professional development and clinical supervision.

The post holder will work as a member of the community therapy team to provide domiciliary rehabilitation as a part of the wider multi-disciplinary service.

Be responsible and accountable for providing specialist assessment and treatment to patients with diverse health needs, who may have complex and / or chronic conditions, in a variety of community settings.

Responsible for the education and training of staff from other disciplines within the multi-disciplinary service and the supervision and delegation of treatment programmes to staff from own and other disciplines

• Work within and adhere to the Chartered Society of Physiotherapists (CSP) Code of Ethics and Professional Conduct, The Health Care Professionals Council (HCPC) standards of practice, and Trust relevant policies and guidelines.
• The post holder is an autonomous practitioner, professionally and legally responsible for provision of highly specialised assessment, treatment, intervention and discharge planning for people referred to the community therapy team.
• Responsible for managing a caseload of clients with a variety of complex conditions including chronic, disabling and life limiting illnesses.
• Assist the team leader, and service manager in the planning, co-ordinating, delivering and evaluating of the Physiotherapy Service provided to the community therapy team.
• The post holder will act as a source of expertise and demonstrate advanced knowledge, skill and experience in Physiotherapy with adults with physical disability within the community, and in supporting the client to live as independently as possible.
• Be proactive representing Physiotherapy with regard to your clinical specialty. This may be locally, regionally or nationally, as deemed appropriate.
• Educate other health care professionals, multi agency professionals and Physiotherapy students in Physiotherapy within the community therapy team
• Work in the community as a lone practitioner in line with the Trust Lone Worker Policy.
• In consultation with the team leader, assist in the day-to-day delivery of the service, including issues relating to annual leave, sickness, continuing professional development and supervision.
• Produce detailed reports regarding patients’ needs including specific intervention
• Adapt practice to meet individual clients’ circumstances
• Ensure that clients and carers where appropriate are involved in the planning and actioning treatment programmes wherever possible.
• Provide spontaneous and planned advice and instruction to relatives, carers and other professionals to promote understanding of the aims of Physiotherapy and to ensure a consistent approach to client treatment.
• Take an active role in liaising with others involved in the care of your clients.
• Be a recognised source of clinical expertise, advice and guidance within your specialty, to provide advice / second opinions when required by colleagues within and outside the department.
