ARE YOU AN EXPERIENCED NURSE READY TO STEP UP TO A SENIOR ROLE?

We are seeking a number of enthusiastic and motivated clinicians to join our ANP team based within seven Community Hospitals across Leicester, Leicestershire and Rutland. We have developed a clinical pathway delivering medical care for patients and our diverse caseload requires practitioners with comprehensive assessment and medical management skills.

This is both a challenging and rewarding role which will give you scope for higher level decision making. Our patient cohort is increasingly complex due to the need to facilitate transfer from acute trusts for individuals who are medically fit for discharge and to prevent admission from home to the secondary care sector. You will be responsible for assessment, diagnosis and development of treatment plans. The wards receive onsite patient review twice a week from a senior doctor and outside of this time the ANP will be the senior clinician on site, leading the ward round and taking clinical decisions alongside the MDT.

The role will bring high levels of responsibility and will also provide the opportunity for autonomous working in this challenging, interesting & changing environment.

Cross cover working may be expected & a base will be discussed at interview. The contracted hours will be worked flexibly between 08.00 - 18.30 Monday to Friday.

It is essential that you hold a UK driving licence & have access to a vehicle. Free parking is available on all sites.

Main duties of the job

MAIN DUTIES OF THE JOB

To work closely with consultant geriatricians, consultant nurses, a team of ANPs, nursing and allied health professionals in both primary and secondary care, to ensure patients receive investigations, interventions and treatment with minimal avoidable delays, whilst improving the quality of care.

To work autonomously assessing individual patient needs, initiating investigations, determining differential diagnosis and initiating appropriate holistic, evidence-based treatment with the development of a personalised clinical management plan, including independent prescribing and appropriate escalation plans.

To have confidence and competence in recognising a deteriorating patient and to initiate a treatment accordingly and appropriately.

Practice autonomously in providing assessment, diagnoses and treatment plans for complex patients including requesting investigations and interpreting results.

To put the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, improving the quality of care.

To provide support and teaching to trainee ANPs and medical students.
