Job summary

We are looking for individuals with Primary Care Nursing skills and experience, and a willingness to develop other skills that they dont already have. This role is integral to ensuring that the practice provides an outstanding level of nursing care.

The hours we would be happy to discuss. The salary is also negotiable based on experience.

Main duties of the job

SCOPE AND PURPOSE OF THE ROLE

To deliver a high standard of patient care as a Practice Nurse in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base

To manage a clinical caseload, dealing with presenting patients needs in a primary care setting

To lead and develop high quality patient care to meet the holistic health care needs of a practice population

To develop the nursing services offered by the practice

Diabetes experience would be desirable.

JOB DIMENSIONS

To manage own clinical workload in general practice responding effectively to patient need and ensuring patient choice and ease of access to services.

The development and use of referral pathways to the local acute NHS Trust services and to Diagnostic and Treatment Centres (where relevant).

To mentor and support other nurses in developing and maintaining clinical skills (where applicable)

We are a busy city centre medical practice offering outstanding patient care, and we have an exciting opportunity for a Practice Nurse to join our Nursing Team to provide primary health care to our patient population.

We are a dual-site practice, based between St Stephens Gate Medical Centre and Newmarket Road surgery, and provide care for over 19,000 patients. We consistently receive excellent patient feedback, and this role is integral to ensuring that the practice provides an outstanding level of nursing care and maintaining this.

You would be joining a practice with a supportive admin team, friendly clinicians and a commitment to developing new ways of working across the whole practice.

Job description

Job responsibilities

Clinical Role

The post-holder will (with appropriate training and competency):

Work independently using own initiative and is accountable for their own professional actions.

Provide support and leadership for other members of the Primary Health Care Team and lead towards the co-ordination of team activities.

Administer adult and child immunisations and vaccinations in accordance with national and

local programmes.

Competent in anaphylaxis and resuscitation techniques.

Ensures safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained.

Offers comprehensive travel health advice for patients prior to travel including malaria prophylaxis, safe sex, food hygiene, sun protection, first aid and emergency medication, health insurance and vaccinations.

Undertake procedures to include, venepuncture, BP management, urinalysis, peak flow, spirometry, new patient health checks, pregnancy testing, stitch/clip removal, ear syringing and cytology, wound management.

Provides a comprehensive family planning service.

Recognises and understands the importance of observing, documenting and communicating the signs and symptoms of child and adult abuse.

Works within all relevant practice and CCG policies and procedural guidelines eg infection control, chaperoning and patient group directions.

Care and Programme Management:

The post holder will

Take a lead in areas such as asthma and COPD management, undertakes the establishment and running of disease management clinics and provides specialist nursing care for the patients or communities that the practice serves.

Initiate the process of diagnosis with patients suspected to have a chronic disease eg diabetes, COPD, asthma, CHD referring to other clinicians as appropriate.

Manages programmes of care for patients with chronic diseases by planning, providing and evaluating care under agreed local/national guidelines, protocols and patient group directions.

Takes lead and manages programmes of care for all areas of patients health including providing and evaluating care under agreed local/national guidelines, protocols and patient group directions.

Maintains disease registers.

Audits outcomes of care against standards and initiates changes as necessary.

Undertakes annual review of patients understanding and ability to self-manage.

Refers to other members of the extended primary health care team eg chiropodist, dietician, specialist nurses.

B. TEACHING AND MENTORING ROLE:

The post-holder will :

  • Promote a learning environment for patients, nurses and other health professionals.
  • Contribute to the planning and implementation of the teaching for existing staff, such as medical students, NP students, GP registrars, Health Care Assistants, within the Practice.

C. PROFESSIONAL ROLE

The post-holder will:

  • Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.
  • Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review
  • Maintain their professional registration
  • Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained
  • Work within the latest NMC Code of Professional Conduct
  • Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards
  • Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation
  • Work collaboratively with colleagues within and external to the practice.
  • Demonstrate leadership
  • Pro-actively promote the role of the NP within the Practice, and externally to key stakeholders and agencies

D. ADMINISTRATION ROLE:

The post-holder will:

Participate in clinical supervision for the nursing team, and act as nurse advisor to other members of the Primary Healthcare team.

Encourage and develop teamwork within the practice.

Participate in multi-disciplinary protocol and Patient Group Directions development

Identify appropriate opportunities to delegate both clinical and administrative tasks to more junior staff.

Help the practice operate in a cost-effective manner.

Identify and manage nursing care risks on a continuing basis.

Participate in practice meetings and practice management meetings, reporting progress as required.

Participate in audits and inspections as appropriate.

Work closely with the doctors and administrative managers in the setting up and/or improving of practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets.
