Job summary

We are looking for a hard-working, experienced, and enthusiastic Primary Care Practice Nurse to help our Nursing team.

The vacancy is part-time, around 20-25 hours per week over 3 days, although we do have some flexibility in total hours and working days. Please state your preferred hours and working pattern in your application.

Main duties of the job

Ideally, you will have previous experience of primary care nursing with the usual treatment room work, injections, smears, dressings, and chronic disease management.Please detail your primary care experience in your application.

You must be professional, courteous, and efficient with excellent people handling skills, enthusiasm and the ability to work under pressure.

Main Nursing duties

  • Provide nursing care to patients as required in accordance with clinical based evidence, NICE and local requirements
  • Appraise wounds and other minor injuries, administer dressings; refer to a practice doctor or the hospital as appropriate
  • Take swabs, cervical smears
  • Provide immunisations and injections to children and adults
  • Assist with Chronic Disease Management using the principles of MECC to maximise encounters and minimise patient visits
  • Support the Nursing team in dealing with clinical emergencies
  • Monitor the blood pressure of patients and consider indications for referral back to the doctor
  • Assist with ECGs and report the findings to the doctors
  • Assist the phlebotomists with venepuncture
  • Insert ring pessaries as directed by the doctor
  • Assist with INR clinics, including patient dosing using INRstar when a suitable Health Care Assistant is not available

Chronic Disease Management

  • Help maintain chronic disease registers
  • Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
  • Coronary heart disease (CHD)
  • Hypertension
  • Smoking cessation
  • Diabetes
  • Asthma
  • Chronic obstructive pulmonary disorder (COPD)
  • Management of other "Best Practice" patient registers
  • Implement and evaluate individual treatment plans for chronic disease patients

General duties

  • Maintain high standards of nursing care to patients of the Practice, and to promote good health care
  • Assist doctors during their surgeries e.g., chaperone, urgent electrocardiograph (ECG) etc.
  • Be responsible for maintaining a high standard of hygiene and cleanliness in the nursing area and of the clinical equipment used
  • Monitor and maintain adequate supplies of drugs, clinical items and equipment
  • Monitor and maintain emergency drugs bag
  • Appropriately recording all treatment undertaken in the patients record on the computer system
  • Ensure templates and read codes are used effectively
  • Ensure maximisation of QOF, Enhanced Services, IIF and other funding streams
  • Obtain and make use of health promotion material in clinics and for display purposes
  • Implement new and revised nursing procedures when necessary
  • Attend meetings, courses and update days as required
  • Use relevant websites for the purposes of supporting the nursing role and giving advice to patients
  • Assist in the support of our HCAs
  • Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
  • Keep up to date with training needs, registration, and revalidation

Health Promotion

  • Develop, implement and embed health promotion and well-being programmes
  • Request pathology services as necessary
  • Discuss diet, alcohol, smoking, and other health problems with patients
  • Family planning
  • Travel advice
  • Cervical cytology; including monitoring and review of the results
  • Assist in different Health Checks

Managerial

  • To work with the doctors and Practice Manager to achieve the best possible service to patients
  • Ensure that the relevant requirements of the GMS Contract are implemented within the nursing department.
  • Monitor, record usage and stock-take vaccines
  • Discuss, implement and audit new nursing procedures
  • Participate in audit and inspection as required by the GMS Contract, including cervical cytology
  • Assist the doctors in the maintenance of relevant and accurate clinical histories
  • Assist in overseeing/supervision of Health Care Assistants
  • Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

Communication

  • Team player and attention to detail skills are essential
  • Help and advise reception/PLT, and other non-clinical, staff on clinical matters
  • Advise patients on the telephone
  • Liaise with the other members of the primary care teams and primary care networks
