Job summary
To support the Care Navigator in the Early Intervention in Psychosis Team to monitor and maintain Early Intervention in Psychosis access and waiting time standards by establishing an maintain systems to ensure standards are met in a timely manner particularly in relation to referrals, risk assessments, CPA, file management and tracking. To support the team with the administration of reports, keeping up to date spreadsheets and data in relation to service user and carer information. 1.To alert the Care Navigator, Team Leader or Manager to any concerns in a timely manner.
2.Support with typing up of care plans and reports for practitioners as agreed by team leader.
3.Support care navigator to develop and maintain databases for performance measures within the team.
4.Discharge and transfer individuals from the service, with the support of the multi-disciplinary team.
