Job summary

Responsible for attending to patients who wish to be seen by a health care professional; diagnosing and treating patients presenting undiagnosed and undifferentiated illness; seeing and advising people in respect of their continuing medical and nursing needs including liaison with internal and external professionals, other agencies to manage the patient effectively and efficiently.

Main duties of the job

The ACP will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner. Appointments will be a mixture of face to face, telephone and visits. There will be a requirement to see extra or emergency patients.

Overall the post holder willwork collaboratively with the general practice team to meet the needs of the patients, delivery of policies and procedures and providing nurse leadership as required.
