We have the brilliant opportunity for two Support Workers to join our Complex Transition Support Service!

This is a permanent, full-time role, Monday - Friday.

The service will work closely with North East and North Cumbria (NENC) Integrated Care Board (IBC) Complex Case Management team. The overall aim for CTSS is to support the discharge of people from acute secondary care inpatient beds / private hospital beds into alternative community settings. CTSS will also assist in the repatriation of people back to the NENC region for those currently residing out of area. The service will provide assessments, discharge planning support and 12 weeks post transition support to settle people into their new accommodation.

About the job

As a Support Worker, you will manage a caseload of service users under supervision. Implement plans for support needs and continually evaluate and monitor, feeding back to key workers, to provide appropriate support and address needs to assist service users to participate fully in community life. You will also provide support to qualified colleagues in the assessment of client need and in the planning and evaluating of their support plans.

Key Responsibilities

  • Manage the needs of service users, carers and partner services to ensure delivery (inclusive of all required resident records) meets all relevant quality standards and is person centred, outcomes focused and protective of individual service users’ dignity and safety.
  • Develop and enact discharge plans and support plans with colleagues within the CTSS and external partners to ensure best possible outcomes for service users / families / carers.
  • Prepare for and join MDT reviews to discuss progress with discharge plans and support plans
  • Coordinate with family members / carers as required to involve them in decision making and care delivery.
  • Support and enable internal and external colleagues to meet the needs of service users who express themselves through difficult or challenging behaviour. Ensure that these approaches are linked to nursing support plans.
  • Deliver care in line with support plans during the initial 12 weeks of a service user being admitted into a new community placement.
  • Recognise changes in behaviour and escalate to CTSS colleagues when support plans are not working effectively or new risks require assessment.

About you 

  • Demonstrable experience of working 1:1 with service users people with complex needs
  • Demonstrable experience of working within health, social care
  • Understanding of recovery in the context of mental health
  • Understanding of the effects of stigma and discrimination
  • Understanding of confidentiality and ‘General Data Protection Regulations’.
  • Understanding of the health and social care landscape
  • Awareness of mental health issues & users’ needs
  • Awareness of behaviours of people expressing difficult emotions, anxiety and anger management
