A
fantastic, exciting, and challenging opportunity has arisen in the Health Case
Management Service. We are looking for a dynamic, self-motivated Band 5 Health
Case Manager Coordinator (HCMC) to join our team. We welcome applications from
health professionals from a wide range of backgrounds.
You will have experience of working with adults and their carers within a
hospital and /or community setting. You will have excellent organisational
skills and be able to work on your own independently and creatively using a
problem-solving approach, as well as part of a busy team. It is essential that
you are motivated, organised, a team player and enjoy the buzz of working in a
fast paced and dynamic environment.
Main duties of the job
The
Health Case Management Service provides a case management function for the
population of Leeds (18 years and above) who are eligible for NHS Fast Track
and Continuing Healthcare funding; this includes hospital and within their own
home, over 7 days, Monday to Friday 08:00 to 20:00 and Saturday and Sunday
08:00 to 18:00. The post holder will -
- Manage
a complex caseload.
- Work alongside MDT including, ICB, Neighbourhood Team and NTHT.
- Support with coordinating of day-to-day tasks and referrals.
- Mentor staff
- Support end of life patients.
The
post will initially be based at Killingbeck Court, Killingbeck; however, the
service is citywide and staff may be required to work flexibly across the city
in the future. Killingbeck as our main base is under review, so the location
will be changing in the near future.

Job Purpose
The post holder will be
responsible for the day to day co-ordination of the teams duty and allocation
responsibilities and will work within their speciality area as an autonomous
practitioner. In doing so, they will assess, plan, deliver and evaluate patient
centred programmes of care to a specialist level ensuring that they practice
within their sphere of competence and knowledge. The post holder will work to
professional and regulatory body codes, standards and guidance at all times
ensuring that their practice is grounded in evidence based theoretical and
practical knowledge.
