Job summary

An exciting opportunity has arisen for an enthusiastic and motivated band 4 Nurse Associate to join the Harrow Community Nursing Team - Central London Community Health Care Trust. The post will be based at in Harrow

The post holder will be joining a well-integrated and established multi-disciplinary team providing nursing care to a varied caseload of patients aged from 18 years old and upwards. And working closely with our Integrated Care services to provide holistic care.

To provide and monitor safe, person centered care to patients in a range of care settings under the direction of a Registered Nurse, without direct supervision in line with an agreed plan of care.

To support the registered nurse in the contribution to on-going assessment, treatment and care planning for patients

To adhere to the Nursing and Midwifery Code of Conduct for Nurses, Midwives and Nursing Associates and work within their scope of practice following the professional standards of practice and behaviours for nurses, midwives and nursing associates and the standards of proficiency.

Main duties of the job

The role of the Nursing Associate is to work with the multi professional team and the Registered Nurse to assess, plan, organise and deliver clinical care in line with service and patient needs, as agreed with supervising clinicians.

The nursing associate has a breadth of knowledge across the lifespan and across the fields of nursing, providing holistic and person centred care and support for people of all ages and in a variety of settings. The nursing associate works independently under the leadership of registered nurses, working within the sphere of nursing and care and within all aspects of the nursing process.   • Registered Nursing Associate with current UK NMC registration
  • Educated to a minimum level 5 Foundation degree level specific to nursing associate practice
  • GCSEs Grade 9 - 4 in Maths and English or equivalent
  • Care Certificate

Experience

  • Previous experience of working within a health / care setting in the field of area applied for
  • Relevant experience of working self-directed or as part of a team
  • Evidence of continuing professional development

Skills & Knowledge

  • Understanding of the scope of the role of the Nursing Associate in context of the team and the organisation, and how the role contributes to service development
  • Understands and acts in line with NMC professional standards for practice contained within the Code.
  • Relevant clinical skills in the area of work

  • Ability to communicate effectively (written, verbal, non- verbal communication) with patients/relatives, carers and other members of the multidisciplinary team.
