Job summary
WE are looking to provide mental health expertise in the care of adults to the community multi-disciplinary team, to ensure service user care is safe and of a high quality. To work in collaboration across professional and organisational boundaries to provide a comprehensive and holistic assessment of our service users needs. To ensure they can safely remain in the community or return home.The Band 6 Mental Health Nurse will support the caseload, they will oversee and manage the risk by carrying out clinical risk assessments and mitigating the risks with a person-centred plan of care.They will take responsibility for developing the mental health knowledge and links between the community teams.
Main duties of the job
The Mental Health Nurse will deliver evidence-based community mental health care, supporting the most complex cases whilst providing support/education/clinical supervision to the wider Community Services teams. Provide leadership to the team on a shift-by-shift basis alongside other Band 6 colleagues.They will promote and maintain the highest attainable standard of nursing care in a community setting, ensuring information on diagnosis and care planning is appropriately documented for continuity and transparency.
