Job summary

The Paramedic Practitioner will work autonomously within the community, using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains and headaches. The Paramedic Practitioner will have a key role in supporting delivery of the care as directed by the PCN in line with the needs of the local population.

Main duties of the job

The role will provide a community Paramedic resource for Health Care Professionals and service users, working in collaboration with other members of the Primary Care Network (PCN) Multidisciplinary Team (MDT). You will be required to work in all GP Practices within the PCN. The role will involve working as an autonomous, accountable Paramedic, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment to deliver quality patient services.

The post holder will use advanced clinical skills to provide education to patients and service users, promoting self-care and empower them to make informed choices about their treatment.

The post holder will assess, diagnose, treat, refer or signpost patients and or service users who attend the surgery with undifferentiated or undiagnosed conditions relating to minor illness or minor injury. The post holder will coordinate telephone triage, taking clinical responsibility for all patients who have requested an urgent appointment.

The post holder will perform any other clinical and clinical administrative duties where trained and determined safe to do so by the organisation. This may include assisting with medical reports, safeguarding, and clinic support.

Job responsibilities

KEY RESPONSBILITIES

Duties will include but are not limited to:

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.

Assess patients presenting with acute illness or for review of chronic disease conditions.

Undertake home visits for acute illness or chronic disease management.

Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.

Perform specialist health checks and reviews; perform and interpret ECGs; perform investigatory procedures as required.

Undertake the collection of pathological specimens including intravenous blood samples, swabs etc.

Support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing).

Provide an alternative model to urgent and same day GP home visit for the network and undertake clinical audits.

Communicate at all levels across organisations ensuring that an effective, patient-centred service is delivered.

Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.

Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Specific Tasks directly related to the post:

Demonstrate highly proficient, co-ordinated, and confident expertise in the delivery of evidence-based care.

Respond to patients with undifferentiated and undiagnosed conditions and undertake assessment, investigation, treatment and care planning for a wide spectrum of patients utilising relevant evidenced based practice, local guidelines and enhanced scope of practice.

Undertake complex assessment of the health and social care needs of patients, analysing these to ensure that the care planned meets the needs of the patient.

Assimilate the assessment and examination findings of other clinicians in order to critically appraise the proposed care pathway and/ or interventions and where necessary provide an advanced lead in determining the overall plan/ pathway.

Formulate and analyse care management plans incorporating a range of approaches, including situations where there is not an established pathway.

Operate as an autonomous advanced practitioner utilising an advanced scope of practice and demonstrating clinical leadership and co-ordination skills in dealing with incidents or events, which may include liaison with ambulance service managers, other emergency services, specialist teams and other health care professionals.
