Job summary
The staff member will be responsible for the care coordination of patients and support the ward and team and have excellent communication skills, be highly motivated and have the ability to respond effectively to arising mental and physical health needs. They will provide sound clinical practice and guidance to junior staff, be adaptable, and capable of delivering a high standard of care.

Main duties of the job
The staff member will be responsible for the care coordination of patients and support the ward and team and have excellent communication skills, be highly motivated and have the ability to respond effectively to arising mental and physical health needs. They will provide sound clinical practice and guidance to junior staff, be adaptable, and capable of delivering a high standard of care.
There is an expectation that they are able to travel within ABUHB to undertake patient risk assessments and support the wider organisation.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
