Job summary

We have an exciting opportunity for PCN Care Co-ordinators to join our Primary Care team. Arrow Health Primary Care Network (PCN) are looking for enthusiastic and highly organised individuals to work within the team providing co-ordination, administrative and support to Plains View Surgery, Westdale Lane Surgery & The Ivy Medical Group. The role will be based within a GP Surgery.

This role will help to deliver enhanced care and support to the local population.

Arrow Health PCN Practice Locations:

  • Daybrook Medical Centre -Salop Street, Daybrook, Nottingham, NG5 6HP
  • Plains View Surgery -57 Plains Road, Mapperley, NG4 1HQ
  • Unity Surgery -318 Westdale Lane, Mapperley, NG3 6EU
  • Westdale Lane Surgery -20-22 Westdale Lane, Carlton, NG4 3JA
  • The Ivy Medical Practice (2 sites) -Apple Tree Medical Practice, 4 Wheatsheaf Court, Burton Joyce, Nottingham, NG14 5EA and Lambley Lane Surgery, 6 Lambley Lane, Burton Joyce, NG14 5BG

Main duties of the job

MAIN DUTIES OF THE JOB

  • Making and managing appointments and making sure that patients have understandable written or verbal information to help them make choices about their care which will address any health in equality.
  • Run reports from clinical systems such as SystmOne
  • Work with teams within the GP practice and those who support GP practices

KEY REQUIREMENTS

  • Good standard of education GCSE or equivalent in English and Mathematics
  • Have ICT skills and knowledge of general software applications relevant to the post, including Word, Excel and management information systems.
  • Proven administrative experience in a complex and demanding environment.

Arrow Health PCN is a group of GP practices (5) that are committed to work together to add additional services and roles to support the workload in GP practices. We cover 41,000 patients within our locality. Our network works closely with all practices and other key members of the network including pharmacies, Citizens Advice, Voluntary Sector and local district and borough councils. Our aim is to continually improve services to patients utilising all available resources.

Our PCN work in partnership with PICS to provide a host organisation for employment contracts, HR and corporate support. The day-to-day work is situated in the GP practices and the everyday line management is undertaken by staff within the Primary Care Network.

JOB PURPOSE

  • A care coordinator works with GP practices to help run reports, organise patient visits, deal with tasks and generally help patients to get a better service from doctors, nurses, clinicians and admin teams within the practice and other professionals.
  • You will be a liaison source of information for the GPs and Practice Managers of the PCN, ICB, NHS England and Primary Care Networks.

KEY RESPONSIBILITIES

  • Run reports from clinical systems such as SystmOne (training will be provided if needed).
  • Book patient appointments.
  • Code clinician and administration contacts.
  • Work with teams within the GP practice and those who support GP practices.
  • Generate reports to help analyse data to understand capacity and demand.
  • Support patients to book appointments, as part of the various projects, programmes and clinical initiatives
  • Help patients manage their needs by answering queries, making and managing appointments, and making sure that patients have understandable written or verbal information to help them make choices about their care.
  • Support or manage clinics as required including management and monitoring of services and staff rotas.
  • Help patients gain access to self-management education courses and peer support/interventions that support them to take more control of their health and wellbeing
  • Support the coordination and delivery of muti-disciplinary meetings.
  • Adhere to organisations policies and procedures, guided by occupational policies and procedures in primary and secondary care.
  • Work without supervision, plan own workload and seek guidance as required from line manager and colleagues.
