Job summary

We are currently seeking an enthusiastic and resourceful Family Support Nurse to provide family support according to individual needs. Within this role you will carry out holistic and clinical assessments of babies, children, young people, and families on your caseload, as well as oversee all caseloads held by family support workers within your location.

Main duties of the job

As a Family Support Nurse, you will provide families with emotional support from the moment they are referred and throughout their bereavement. Including providing information when needed, signposting to other agencies and liaising with other professionals on their behalf. You will also hold the responsibility of directly line managing Family Support Workers holding caseloads within your location. This role will require you to work and travel within the areas of Derbyshire, Nottinghamshire, and Lincolnshire, both inner city and rural outskirts depending on where the families are located, as well as to Rainbows Hospice in Loughborough for team meetings and events. Hours of work would be 37.5 (full-time) between Monday and Friday with some Saturday working on call and for any events such as Open Day.

We want to bring the Rainbows experience to families, wherever they are located within the East Midlands not just at our Hospice in Loughborough. Bringing the care and facilities into family homes and through community hubs. About the Role To support the Families on your caseload within the identified locality of Derbyshire, Nottinghamshire, and Lincolnshire. Some of the key responsibilities include (but not limited to): To undertake the initial holistic assessment of babies, children, young people, and their families, newly referred, for suitability of accessing family support service and/or other hospice services. To provide ongoing clinical and holistic assessment and specialist palliative advice. To act as a resource and provide knowledge, support and information to the family support workers and wider family support team. To ensure that babies, children, young people, and their parents are empowered through information and understanding of their rights and options to access services required. Requirements: As an experienced Nurse, youll need to be a qualified RSCN/RN (Child Branch)/RNLD/RGN with post registration experience working with babies, children and/or young people with palliative/complex care needs. It is essential that you have experience working with families, offering emotional and/or bereavement support as well as effective communication skills. It is desirable for the ideal candidate to have experience in managing their own caseload and direct line management as this role with require you to manage a team of Family Support Workers and oversee their caseloads, providing support and guidance where needed. It is essential for you to have your own mode of transport as you will need to travel across the areas of Derbyshire, Nottinghamshire, and Lincolnshire, sometimes to rural locations of Familys homes with no access to public transport.
