This is an exciting opportunity to join our
integrated community nursing team and be part of our evolving community model
providing advice and support to patients in their own homes.
We have part time and full time hours
available for ACNS posts (18.75 and 37.5 hours)
You will join a supportive, patient focused team
who are dedicated to making a meaningful difference and going that extra mile
for the people of Hastings & Rother.
We are looking for someone who is
passionate about palliative care and has transferable nursing skills and
experience. You will be able to build a natural and trusted rapport with
patients and families and provide compassionate care and support to those who
need it most.
Our ideal candidate has excellent communication skills, sound
judgement, and a real commitment to patient-centred interdisciplinary team
working. You will be able to openly and honestly support people with sensitive
topics including loss, grief and dying.
While palliative care experience is
desirable, it is not essential to join our Hospice team.

Main duties of the job
To
support the holistic care of people with specialist palliative care needs in
the community through the provision of expert palliative care assessment and
management, in line with their personal priorities and goals.
To
act as a specialist resource for the patient, their families and the wider
health and social care team in the support and care of people with life
limiting illness including physical, psychological, social or spiritual support
needs.
To develop the
skills required as a senior member of the Integrated Clinical Team, to play an
active role in clinical governance, clinical service development, audit and
research, including leadership of clinically facing projects.
To work in an integrated way with external
colleagues, fostering strong working relationships and effective communication
to ensure seamless support and care for patients and families. To actively contribute to
the education and professional development of internal and external colleagues
in line with area of specialist palliative care knowledge and experience.
To support 7-day service provision and participate in
the Clinical On call rota
