Job summary
Neighbourhood Coordinators provide a key role coordinating the activity of Neighbourhood Teams - made up of staff from across the health, care, housing and community and voluntary system working collaboratively to improve the health and wellbeing of the most vulnerable and complex people in their local population .
There are two positions available. The role based in Louth is part time at 22.5 hours over 3 days per week and the role based in Boston is full time at 37.5 hours per week.
The role in Boston will work collaboratively with Boston Primary Care Network to also help coordinate the work of their Health and Wellbeing Team working in the Neighbourhood.
Main duties of the job
To maintain and facilitate and provide first point of contact for a network of stakeholders working across the neighbourhood.
To have oversight of all cases nominated for neighbourhood working - including direct referrals and cases and cohorts flagged to the team for collaborative support.
To coordinate, record and track actions from cross system multi disciplinary meetings including regular and bespoke MDT meetings
To develop , maintain and share a knowledge base and liaise with stakeholders to help ensure the best response and best possible outcomes for people
