Job summary

Health & Wellbeing Coaches support pts to take proactive steps to improve the way they manage their physical & mental health conditions.They support pts to develop their knowledge, skills & confidence or to build their patient activation;in managing their health & care, to improve their health outcomes & quality of life. Coaching & motivating patients through multiple sessions to identify their needs, set goals, & supporting patients to achieve their personalised health & care plan objectives & providing interventions such as self-management education & peer support.You will manage a caseload of patients, according to the needs, priorities and support required by individuals.

Main duties of the job

Provide 121 & group health coaching support for people with 1 or more long-term conditions,with the aim of: improving peoples levels of activation, empowering people to manage their own health & improve their health outcomes. It is vital that you have a strong awareness & understanding of when it's appropriate or necessary to refer people back to other health professionals, when the needs are beyond the scope of the health & wellbeing coach role e.g. when there is a mental health need requiring the patient be referred to an appropriately qualified practitioner.Support network initiatives to improve staff wellbeing, including the Better Health at Work programme.Build & develop strong working relationships with health, social, & third sector providers in the Durham Dales, including GP practices & the local Wellbeing for Life service.Work closely with professionals from a variety of health specialties.

About us

You will be working as part of the Bishop Auckland Primary Care Network (PCN) team to support the patients of 3 GP practices (Station View Medical Centre, Auckland Medical Group and Bishopgate Medical Practice).The PCN is recruiting a number of new roles to support the wellbeing of patients in this mixed urban & rural area, & you will be part of this proactive & expanding team.You will work flexibly, supporting patients through 121 & group sessions delivered in GP practices, patients homes, & via online video consultations.The PCN is committed to training & development of all staff & you will be able to develop your career in line with your personal & professional objectives. They will be proactive with a flexible attitude, keen to work as part of a team & committed to developing themselves in this skilled role & will support colleagues to do the same.Work flexibly, adapting to the needs of the service & client group.

  • Be willing to attend training with a non- clinical SSM health coaching skills programme (minimum 4 days)
  • Health related qualifications minimum level 3 e.g. Health and Well-being Coaching, nutrition, weight management, personal training, diabetes

Desirable

  • Coaching/counselling qualification/ experience or other relevant qualification/experience involving reflective listening skills relevant training and experience in non-clinical SSM Health Coaching through a PCI- accredited organisation
  • Personalised Care Institute (PCI) accredited trainer or organisation

Experience

Essential

  • Experience of using coaching approaches/frameworks and models or other similar helping strategies e.g. Motivational Interviewing
  • Experience of working in a multidisciplinary team
  • Experience of working in health and social care care/community development setting or similar

Desirable

  • Experience of successful partnership working across statutory, voluntary and community sector
  • Experience of working in a primary care setting
  • Networking experience
  • An understanding of the biopsychosocial model of health and the social determinants of health.

  • Able to work within a biopsychosocial model, using a range of tools and techniques to enable and support people, such as agenda setting, goal setting, problem solving
