Job summary
Do you have the skills, experience, and desire to
support and influence the health of a community and its patients to tackle
health inequalities? We are looking for a Nursing Associate to do this role as
part of a Primary Care Network (PCN) multi-disciplinary team. The post holder
will mainly focus on supporting patients with multi-morbidities such as
diabetes, chronic kidney disease and hypertension. They will be responsible for
ensuring the effective holistic provision of high-quality nursing care to
people in a clinic and outreach environments who are registered with practices
in Burmantofts, Harehills and Richmond Hill.
Main duties of the job
This exciting role with see the Nursing Associates
role actively contribute to a patients health and wellbeing through the
delivery of holistic care, a whole-person approach which considers, and equally
values, physical, psychological, public health needs, social economic, spiritual,
and other factors in the assessing, planning and delivery of care. Providing
all relevant competencies (to be developed in line with the requirements of the
service) have been achieved, they will have delegated responsibility for a
pre-agreed caseload under the supervision of a nominated clinician within the
PCN multi-disciplinary team. The post holder will be integral to the team of
clinical pharmacists, specialist Mental Health HCA, care coordinators: for
mental health, care home patients and children, GPs, Nurses, Health and
Wellbeing Coaches, Social Prescribers, and the wider local care partners such
as Community services, hospitals, specialists for long term conditions along
the patients pathway.

known priority for BHR PCN. We think this is a
challenging but extremely rewarding opportunity for a nursing associate to join

Our population have a higher prevalence of mixed mental and long-term
conditions because of the nature of our more deprived communities, particularly
at an earlier age. Along with our culturally diverse population, these
multimorbidity patients are typically our hard-to-reach patients.
