Job summary
This service covers an enhanced model of care to provide wrap-around care for children and young people in our acute hospitals and 136 suites for Surrey young people, 12 hours a day, 7 days a week. The team builds relationships within the acute hospitals to further support care for the young person holistically (emotional, mental, and physical) and to improve communication between the teams involved.
As part of the EWMH Team, you will engage and build therapeutic relationships with the young person's family or caregiver and advocate on their behalf to provide a collaborative care plan and risk assessment, make recommendations to the care plan to meet the needs of the young person, and ensure acute hospital colleagues are able to contribute and are aware of the young person's care needs.
You will participate in the provision of assessments, care planning, and treatment for children and young people who may have presented with mental health needs, some of which include disordered eating due to neurodevelopmental issues or challenging behaviours. You will be part of a specialist team supporting the paediatric wards across Surrey.
This is an 18-month fixed-term contract that may be extended with the success of the pilot service. There will also be opportunities within our crisis pathways at the end of this term.
Main duties of the job
- Undertake holistic assessments, including the risk of the child, adolescent, or family needs, either individually or in identified complex cases jointly with another member of the EWMH team.
- To liaise with social care, education, and other appropriate agencies, professionals, and caregivers to ensure the best outcome for the child, adolescent, and family.
- Act as a key worker for children and young people following the assessment. Developing an appropriate plan of care.
- To be part of the EWMH rota.
- Co-work with other members of the team, including the paediatric team on the ward.
- Take responsibility for maintaining accurate and timely records of all work undertaken in accordance with SABP Trust policies and legal requirements.
- Have basic IT skills and data entry skills to enter clinical information into the databases.
- Contribute to the overall standard of services provided by the team.
- Promote effective liaison with all statutory and voluntary agencies concerned with young people and mental health.
- Ensure high standards of care are provided and that regard for the dignity, individuality, self-esteem, and wellbeing of individuals and their families is given.
