We have an exciting opportunity for a motivated individual to join our Patient Flow Team. It is a great time to be a part of this team as we expand our offer to all of mental health inpatient services. As a part of this team you will be supporting all patients to ensure their discharge is managed effectively, preventing any unforeseen barriers to discharge.

Main duties of the job
The post holder will have oversight of all referrals in to our inpatient areas and support the team in identifying and prioritising admissions in to beds. You will have the ability to form and maintain strong working relationships with colleagues both internally and externally to support all patients to access services and to provide them with the best experience through their discharge planning.
The post holder will have direct line management of roles within the patient flow team. There is an expectation the day to day provision of bed capacity is maximised as well as attending and chairing meetings which support timely discharge. Ensuring the delivery of a clinically effective service which meets the needs of
individuals, families and communities in line with local and national development
plans, Trust policies and procedures.
Communication in this role is key to success. There is an expectation we all have a shared vision and goal for our patients in order to provide them with the best experience and outcome.

users is delivered in accordance with LPT Trust standards.
