Job summary

Brynhyfryd Medical Centre is looking to recruit a Practice Nurse . We are a growing practice with a large and diverse population.

We are looking for a Practice Nurse to join our friendly practice, serving a population of over 10,000 patients.

The successful candidate will enjoy working as part of a dedicated team, with the added satisfaction of working for an organisation committed to the care and well being of others.

Main duties of the job

You will be supporting patients to be healthy, by monitoring long term conditions, and focusing on health prevention and screening.You will be working alongside a team of GPs, a Physician Associate, an Associate Practitioner and a Health Care Support Worker.

The post holder will provide and maintain a high standard of nursing care for patients. Providing assistance to the doctors and other members of the primary health care team. The post holder will run chronic disease clinics and act on their own initiative in dealing with day to day issues, as usually undertaken by a RGN nurse.

Job description

Job responsibilities

To manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems

Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccine schedule

Treat patients with minor injuries

Provide directly and assist GPs with contraceptive services

Undertake health promotion to help educate patients and assess problems presented opportunistically by patients

Select, carry out and interpret results of specific tests

Take ECGs

Simple wound management

Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, safe sex, food hygiene, sun protection.

Able to perform cardio-pulmonary resuscitation according to current UK guidelines

Ensure infection control and cold chain guidelines are maintained.

Ensures awareness of statutory and local clinical protection procedures, including Safeguarding Children

Assist with the management of stock and equipment to include refrigeration, sterilisation and emergency equipment.

To maintain accurate patient records and document consultations according to NMC guidelines

To maintain own annual registration with the NMC and adhere to Code of Professional Conduct

Report all untoward incidents and/or accidents

Within the scope of the role contribute to and support new ways of working within the team

Follow policies and procedures within own role
