Job summary

Millbrook Medical Centre is a well-established and busy training practice serving a population of just over 11,900 patients from two sites across Kirkby. We currently have a fantastic opportunity for a Band 7 Clinical Pharmacist to join our experienced and friendly team.

The post holder will be responsible for working closely with both the clinical and non-clinical teams to deliver a high standard of pharmaceutical services to all healthcare professionals and patients.

You will be given daily support from our multi-disciplinary team of professionals including two further Pharmacists and Pharmacy Technician. We also have a 5 strong management team and 23 strong admin team and operate a Supervising GP model.

The post holder will be an integral part of the general practice team as well as part of a wider pharmacy network and multi-disciplinary team and will ideally have at least two years post qualification experience.

In return we will give the post holder support to become an independent prescriber as well as opportunity to develop any area of special interest.

Main duties of the job

To participate in person-centred medicines optimisation and quality improvement.

To participate in multi-disciplinary meetings to optimise patient care using expert knowledge of medicines for specific disease areas.

Be responsible for proactively managing people with complex polypharmacy.

Participate in the pharmaceutical management of people with chronic diseases both face to face and via telephone.

To manage a therapeutic drug monitoring system and the recall for patients taking high risk drugs.

To participate in the organisations medicines optimisation systems, including the repeat prescribing and medical review systems.

Develop and deliver educational programmes for healthcare professionals and patients in order to improve patient and carer understanding, confidence in and adherence with their medication.

Our Practice is led by 4 experienced GP Partners incorporating a 10 strong planned care team consisting of 2 further Pharmacists, 1 Pharmacy Technician, 2 Practice Nurses, 3 Health Care Assistants and a Phlebotomist.

Responsibilities:

    • To provide clinical expertise on person-centred medicines optimisation and quality improvement.
    • Working as a team develop and implement clinical pharmacy services.
    • Contribute to integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
    • Patient Care:

    • Participate in multi disciplinary meetings to optimise patient care using expert knowledge of medicines for specific disease areas.
    • Conduct comprehensive medication reviews and interventions for patients.
    • Be responsible for proactively managing people with complex polypharmacy.
    • Contribute to the care management of patients with chronic diseases.

    • To provide medication review services for patients in the practice and during domiciliary visits to the local nursing home.

    • To deliver long term condition clinics and home visits, particularly for patients with complicated medication regimes, and prescribe accordingly.

    • Quality Improvement:

    • Work as a team to drive quality improvement initiatives related to medication safety and efficacy.

    • Work as a team to provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the practice to help tackle inequalities.

    • To contribute to public health campaigns (e.g covid-19 or flu clinics) through advice or direct care.

    • Education and Training:

    • To support junior clinical pharmacists and pharmacy staff.
    • Participate in delivering educational programmes for healthcare professionals and patients in order to improve patient and carer understanding of confidence in and compliance with their medication.

    • To undertake all mandatory training and induction programmes.
