Job summary

Albert Road Surgery are recruiting for a Digital Transformation Lead who will work closely with the PCN Clinical Director, PCN Manager, and individual practices within the network to improve patient access, improve efficiency, increase patient and staff satisfaction and build sustainability of the PCN and practice services.

Location: NewhamJob Title: Docklands PCN Digital Transformation LeadSalary: £38,890 Location: Docklands Primary Care Network, Newham (E16 Health, Tollgate Medical Centre, Royal Docks Medical Practice)The successful candidate would be expected to work collaboratively within the network to understand current service models, identify challenges, and agree priorities for improvement. It will be essential for the post holder to liaise with other PCN Digital and Transformation leads and key individuals within the GP Federation and the ICB including IT / Digital Leads.Using data quality improvement (QI) processes and optimising digital tools and innovation, the post holder will lead on planning and implementation of operational process to support PCN priorities which will improve the delivery of primary care services locally.to support the delivery of transformation across the PCN through operational processes, pathways embed transformation and change.The ideal candidate will need to be proactive, have a passion for Primary Care and have an understanding of the NHS digital landscape to assist the PCN with its digital transformation agenda.This is an exciting opportunity to lead transformation work across the PCN and use digital technology where appropriate to enable change and maximise use of available tools.Main duties of the jobProvide oversight and accountability to the delivery of PCN contracts and specificationsAssist with collation of financial information to support strategic funding decisionsMonitor and check the accuracy of submissions for the DES and other contractual requirements.Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase PCN productivityBuild relationships and facilitate collaboration between practices and the wider system to support healthcare deliveryReview and improve the PCNs digital maturityUse data and improve its quality to understand demand, capacity, and activity and achieve national targets for Digital andamp; TransformationDrive improvements in patient experiences of accessSupport population health managementFacilitate clinically-led innovation and the effective adoption of improvement initiatives, including integrated working at neighbourhood level to improve access to services for patientsTo be considered for this exciting role you must have: GCSE grade A to C in English and MathsEducated to degree level or equivalent experience.Evidence of continued professional developmentProject Management qualification or equivalent experienceExperience managing some or all of the following: people, data, projects, service improvement, service delivery.Experience implementing policy.Experience within the NHS or public sector.Experience managing projects and implementing change.Ability to deliver multiple projects against tight deadlines.Experience within primary care general practice, community care or commissioning
